This has been bothering me for some time. I think it would be great if when I attached an image to my post and included it inline, that when somebody clicked on, a new tab would pop up in their browser with the full-size image. Unfortunately the attachment tag's BBCode is not editable like other tags (such as anchor, audio, etc., which we have made recent improvements to).
This solution worked for me:

In order to get the needed file ID before posting, I had to click "Preview" and then open the browser dev tools.
Demonstration:
[url=http://forum.sagittal.org/download/file.php?id=134] [img]http://forum.sagittal.org/download/file.php?id=134[/img] [/url]
results in:
I did also have to cancel out of my original post and start a new one in order to avoid the duplication of the image, once in its non-inline attachment form, and once as the img tag.
The orange underline due to the url tag is suboptimal, but it'll do.
